Information shall be gathered in each of the four data categories. To qualify for services, at least one of the criteria must be met by a score on a nationally normed standardized test. Any data used in one category to establish a student's eligibility may not be used in any other category. Assessment data must be current within two years.  Students must meet THREE OUT OF THE FOUR AREAS of eligibility.  If a student's name is submitted for review a team of gifted endorsed teacher leaders, a counselor from submitting school and one of the student's teachers' will assess all the information and make a determination within a reasonable time frame.  This is done only after a parent/guardian and teacher fills out, signs, and turns in a characteristics and permission form.

Parent permission forms must be completed and signed in ink before any student can be assessed by the MCSD Gifted Education Department. The original form must be turned in.  No copies or faxed copies can be accepted.

3rd -12th grade referrals must have copies of the last two years of report cards attached to referral before assessment will be considered.
